Why
should I adjust my insulin?
If
your blood glucose stays within normal to goal range with your current insulin
dose then you probably don't need to change. Some reasons you might want to
adjust your insulin dose could be:
You
want Flexibility in the time you eat and the foods you choose.
You
want to be able to change your insulin dose if your blood glucose is too
high or too low.
Which
insulin should I adjust?
| Out of range Blood Glucose Value (Not explained by food, illness, exercise, stress) | Pattern Adjustment* (Start by adding 1-2 units. Watch your blood glucose for 2-3 days before making another change. It is better to change 1 dose at a time.) | |
| If your Fasting blood glucose is | high | Increase PM or bedtime N/L |
| low | Decrease PM or bedtime N/L | |
| Before Lunch | high | Increase morning short-acting insulin |
| low | Decrease morning short-acting insulin | |
| Before Dinner | high | Increase morning N/L or lunchtime short-acting |
| low | Decrease morning N/L or lunchtime short-acting | |
| Before bedtime | high | Increase dinner short-acting |
| low | Decrease dinner short-acting | |
| During the night | low | Decrease evening N/L or move dinner N/L to bedtime |
How
much should I adjust my insulin at one time?
If
possible, adjust only 1 insulin at a time. If your blood glucose is out of
range at several points during the day, start by correcting your fasting
blood glucose. Once corrected, move to correcting the lunchtime reading.
If
you are adjusting your short-acting insulin (H or R) before meals, you can
use the 1500 rule to determine how many blood glucose points will be covered
by 1 unit of H or R.
If
you are adjusting your long acting insulin (N, L, UL), you might want to
start with 1 - 2 units of insulin, added to- or taken away from your current
dose. Wait 2 - 3 days to see if this insulin adjustment worked before
changing again.
